Shared Living Provider/Contractor

The Shared Living Provider Contractor, under the supervision of the Residential Service Coordinator, is responsible for direct care and instruction of individuals receiving residential services. The job emphasis is on giving the client the opportunity and instruction necessary for them to be as independent as possible for activities which occur in their own lives.

Principle Duties and Responsibilities

  • Implement, modify, and report on programs for consumer skill acquisition according to agency policies and procedures.
  • Attend to and monitor client needs as identified in the IPP
  • Administer medication as prescribed, following doctor’s orders and utilizing the correct agency procedures. Ensure clients attend needed doctor and dental appointments.
  • Assist with client financial needs which may include entering transactions, establishing and balancing accounts, insuring bills are paid and budgeting income in lieu of expenses on a timely basis.
  • Assist with the clients’’ daily living needs which may include grooming and hygiene needs, grocery purchasing, menu planning, household management, cooking, care of clothing, etc.
  • Attending to the clients’ community access needs which may include planning and organizing recreational activities within appropriate social environments, providing/arranging transportation.
  • Effectively communicate as needed with parents, guardians, supervisor and other agency staff on the status of the clients, reporting observed barriers no mentioned in the IPP to the Residential Service Coordinator.
  • Act as an appropriate role model for the consumers according to the agency’s philosophy. Maintain a harmonious and harmonious atmosphere in the residential setting.
  • Display professionalism which includes meeting timelines, maintaining as appropriate and professional manner in all areas related to the consumers and agency. Submitting quality written work products.
  • Attend staff meetings, training's, and accompany client to medical appointments as requested by the team.

Educational and Experience:

  • Must be a minimal age of 18
  • Must have a high school diploma or GED
  • Must pass a background investigation which includes a criminal history and an APS/CPS check
  • Must have at least 1 year of experience doing similar work

Knowledge, Skills, & Abilities

  • Ability to communicate clearly, both orally and in writing
  • Ability to read and understand and effectively utilize written materials and directions.
  • Ability to perform basic math functions, i.e., adding subtracting, multiplying and dividing.
  • Knowledge of general home management skills with ability in the following areas: cooking, housekeeping, menu planning, household shopping and laundry.
  • Knowledge of the community and modes of access including recreational opportunities.
  • General Knowledge of basic health care procedures including personal hygiene, general first aid practices, routine dental and medical care.
  • Understanding of human development and needs on a general and individual level.
  • Ability to autonomously make sound judgments and use common sense.

Job Types: Contract, Full-time

Salary: $60,000.00 - $90,000.00 per year
